By submitting this form, I agree that this Booking Request Form is used by Berchman Paul Jr for the purpose of coordinating agendas. All expenses will be evaluated for the event in question in accordance with the information provided in this form. Submission of this form is only a request and DOES NOT CONFIRM the participation of Berchman Paul Jr in said event. Correct information is required to process this request. After review, Berch or a representative will make contact with you.

Subject to the needs of the request, each event may require any or all (but not limited to) of the following: A 20% deposit, travel expenses, lodging, and meals. In case of confirmation, more details will be provided and included in contracts, commitment letters, and technical riders.

CANCELLATION POLICY

Once this event is confirmed, the date is reserved exclusively for your organization. If the event is canceled within 30 days of the scheduled date, 50% of the agreed honorarium is due. If the event is canceled within 7 days of the day of the event, the full honorarium is due, as 1127 Collective has already committed that date and declined other ministry opportunities.
